Application

Every
application for a driver's, motorcycle operator's license or endorsement shall be made
upon the approved form furnished by the Registrar of the Bureau of Motor Vehicles.
This
application shall state the social security number if such has been assigned. If the
applicant has never had a social security number assigned, he/she may obtain a license
without the social security number.

Every
application for an identification card or duplicate shall be made on a form furnished by
the Registrar of the Bureau of Motor Vehicles.
The ID
application shall state than an applicant is not required to furnish the
applicant's blood type. It is the option of the applicant as to whether they wish to have their social
security number on the application for the Identification Card.
