Medical Procedures for Temporary Permit

Medical Procedures for Applicant Applying for a Temporary Permit

The applicant for a temporary permit is asked the medical status questions on the application. The following procedures must be met if the applicant states that he/she has a medical condition that possibly could or would not affect his/her driving.

  1. Medical Packet (BMV 2058) is issued by examiner.

  2. Applicant must return completed medical form to exam station within 30 days.

  3. Examiner retains the exam form.

  4. Examiner mails (BMV 2003) (to include vision and written scores) to BMV in 2341 envelope.

Applicant returns medical packet to examiner who checks form to determine if it has been completed and contains a physician's statement. If form is not completed in its entirety, the examiner returns the form to the applicant to have it completed. See completion of medical packet.


Medical Procedures for Applicant Converting a Valid Out-of-State License


After completing the vision and written examinations applicant is asked medical status questions. The procedures outlined above will be followed if the applicant states that he/she has a medical condition that possibly could or would not affect his/her driving.  Since the applicant had been the holder of a valid out-of-state license, one additional procedure is added: Applicant is given a BMV 2015 (provides driving privileges within Ohio for 60 days).

Applicant returns medical packet to examiner who checks form to determine if it has been completed and contains a physician's statement. If form is not completed in its entirety, the examiner returns the form to the applicant to have it completed. If applicant is the holder of an expired BMV 2015, he may be issued a second one good for 20 days.  See completion of medical packet.
