Acceptable Identification Documents

Driver License Rules & Regulations
Section 4501:1-1-21


In order to obtain any Driver License, Commercial Driver License CDL, State of Ohio Identification Card or a duplicate of any of these items, the applicant, and any co-signer for a minor applicant, must present two (2) documents that satisfy proof of both: (1) Name and Date of Birth and (2) Social Security Number, if ever assigned. 

 

The primary document must contain the full name and date of birth and must be verifiable.  The Social Security Number must appear on either the primary document or the secondary document.  The name on the documents must match the person's current name unless a change of name is established by a court order, Marriage Certificate, Marriage License or Divorce Decree.  The court order may be a primary or secondary document. Only original documents or a photocopy bearing an original certification by the issuing authority is acceptable.  Uncertified copies or copies of certified documents are not acceptable unless otherwise noted.  Failure to provide two (2) acceptable documents to prove both date of birth and Social Security Number, if ever assigned, shall result in the denial of the application.

 

Note: At least one primary and one secondary document must be provided by the applicant.

 

This includes those persons who have the driver license or identification card in their possession. The  driver license or identification card is acceptable as one (1) of the documents as long as it has not expired beyond six months.

  4. A copy bearing an original certification by the issuing authority is acceptable.

  5. Uncertified copies of certified documents are unacceptable.

  6. Failure to provide 2 acceptable documents shall result in the denial of the application.
