To apply for this specialty plate, use the form VTR-56 (Application for a Parade License Plate).
Instructions for Downloading Forms
Most forms are provided in both PDF and Microsoft Word Template 2000 (.dot) formats. PDF documents may be viewed
using Adobe Acrobat Reader.
For PDF Format
Click on the PDF file you wish to download. File will open in PDF Reader. If you do not have
Adobe Acrobat Reader, free downloads are available
at Adobe Acrobat Reader.
For Word Format
1. Click on the .DOT file you wish to download. You will be given the option to save the document to
the directory (folder) of your choice. Once you have chosen a directory the file will be download to your
computer with an .EXE extension.
2. Open Windows Explorer (File Manager) and double click on the downloaded .EXE file.
The .EXE file will be unzipped and appear as a .DOT file in the same folder.
3. You can then open the .DOT file through Windows Explorer or MS Word.
Faxed Copy
To obtain a faxed copy of a form, call toll free 1-888-232-7033 and enter the correct fax form number
and a return fax number when prompted.
Any fax form presented for processing must be legible and complete. Original Signatures are required on application forms.
The department will not accept an application received by fax.
If you are unable to download any of the following forms, or would like a vehicle registration and titling form not listed,
please contact our Customer Help Desk at (512) 465-7611, and they will be glad to have the form mailed or faxed to you.
For other forms, visit your local county tax assessor-collector or
a Vehicle Titles and Registration Regional Office.
