Former military plates are issued to vehicles including trailers, regardless of the vehicle’s size, weight or year of manufacture that was manufactured for use in any country’s military forces and is maintained to accurately represent its military design and markings. The vehicle must be used exclusively for exhibitions, club activities, parades, or other functions of public interest and not used for regular transportation. The vehicle may not carry advertising.

Applications must be filed with the county tax assessor-collector. The specialty license plate fee for the five-year period is: vehicles manufactured in 1921 and subsequent years $50.30; vehicles manufactured in 1920 and prior years $40.30. These fees are prorated yearly. Of each specialty license plate fee collected, $.50 is sent to the county in which the owner resides.

Instructions for Downloading Forms

For PDF Format
Click on the PDF file you wish to download. File will open in PDF Reader. If you do not have Adobe Acrobat Reader, free downloads are available at Adobe Acrobat Reader.

For Word Format
1. Click on the .DOT file you wish to download. You will be given the option to save the document to the directory (folder) of your choice. Once you have chosen a directory the file will be download to your computer with an .EXE extension.
2. Open Windows Explorer (File Manager) and double click on the downloaded .EXE file. The .EXE file will be unzipped and appear as a .DOT file in the same folder.
3. You can then open the .DOT file through Windows Explorer or MS Word.

Faxed Copy
To obtain a faxed copy of a form, call toll free 1-888-232-7033 and enter the correct fax form number and a return fax number when prompted.

Any fax form presented for processing must be legible and complete. Original Signatures are required on application forms. The department will not accept an application received by fax.
