spacer spacer spacer Operation Iraqi Freedom License Plate

Two plates are issued to passenger cars or light trucks and one plate is issued for motorcycles to applicants who served in the United States armed forces on or after November 8, 2002, or on or before May 1, 2003. The applicant must be honorably discharged if no longer a member of the United States armed forces. The application is submitted to the Vehicle Titles and Registration Division.

No fee is charged other than the regular registration fee and other applicable fees. These fees are submitted to the county tax assessor-collector’s office when the specialty license plates are approved. Qualified applicants may obtain additional sets of the same type of specialty license plates for the regular registration fee and other applicable fees. The surviving spouse of a person who would be eligible for Operation Iraqi Freedom specialty license plates is entitled to continue to register one vehicle with the specialty license plate as long as the spouse remains unmarried.

Instructions for Downloading Forms
Most forms are provided in both PDF and Microsoft Word Template 2000 (.dot) formats. PDF documents may be viewed using Adobe Acrobat Reader.

For PDF Format
Click on the PDF file you wish to download. File will open in PDF Reader. If you do not have Adobe Acrobat Reader, free downloads are available at Adobe Acrobat Reader.

For Word Format
1. Click on the .DOT file you wish to download. You will be given the option to save the document to the directory (folder) of your choice. Once you have chosen a directory the file will be download to your computer with an .EXE extension.
2. Open Windows Explorer (File Manager) and double click on the downloaded .EXE file. The .EXE file will be unzipped and appear as a .DOT file in the same folder.
3. You can then open the .DOT file through Windows Explorer or MS Word.

Faxed Copy
To obtain a faxed copy of a form, call toll free 1-888-232-7033 and enter the correct fax form number and a return fax number when prompted.

Any fax form presented for processing must be legible and complete. Original Signatures are required on application forms. The department will not accept an application received by fax.
