Subject:	Revised Applications for Mobile Amateur Radio Operator License Plates (Form VTR-53) and Volunteer Firefighter License Plates (Form VTR-311)

PURPOSE
To advise you that the Application for Mobile Amateur Radio Operator License Plates (Form VTR-53) and the Application for Volunteer Firefighter License Plates (Form VTR-311) have been revised (see attached).

DETAILS
The Texas Department of Transportation (TxDOT), Vehicle Titles and Registration Division (VTR), has revised the Forms VTR-53 and VTR-311 to include the following:

APPLICATION FOR MOBILE AMATEUR RADIO OPERATOR LICENSE PLATES (FORM VTR-53)	APPLICATION FOR VOLUNTEER FIREFIGHTER LICENSE PLATES (FORM VTR-311)
The information regarding Replacement License Plates has been removed. The information regarding the $2.00 fee for duplicate license plates has been added on back of application.	The Driver’s License Number field has been added. The official responsible for verifying that the volunteer firefighter has satisfactorily completed the required hours for Basic Volunteer Firefighter has changed to the Texas Commission on Fire Protection, or the State Firemen’s and Fire Marshals’ Association of Texas.

COUNTY ACTION
A copy of the revised form is attached. The application may be photocopied for use until you receive a supply of the revised forms. The old forms will continue to be accepted. This form is also available on the TxDOT Web site at www.dot.state.tx.us.
