Subject:	Revised Application for Peace Officer License Plates (Form VTR-406)

PURPOSE
To provide you information regarding the revised Application for Peace Officer License Plates, Form VTR-406 (Revised 12/2003).

DETAILS
The Texas Department of Transportation (TxDOT) has revised the Form VTR-406 to include the following changes:

Driver’s License Number field has been added.
Information regarding documentation that applicant is required to submit to the department in order to obtain the Peace Officer License Plates has been moved to the back of the application.

COUNTY ACTION:
A copy of the revised form is attached. The application may be photocopied for use until you receive a supply of the revised forms. The old forms will continue to be accepted. This form is also available on the TxDOT Web site at www.dot.state.tx.us.
