These documents contain text of correspondence sent by TxDOT to the county tax assessor-collectors, statutory agents who provide vehicle registration and titling services for citizens in Texas. This information is provided to apprise the general public of issues affecting the state’s vehicle titling and registration process.


Registration and Title Bulletin #116-03

Date: October 8, 2003
To: All County Tax Assessor-Collectors
Subject: Revised Application for Disabled Person Identification Placard and/or Disabled Person License Plate, Form VTR-214.

PURPOSE
To inform you of revisions made to the Application for Disabled Person Identification Placard and/or Disabled Person License Plate, Form VTR-214 (Revised 8/2003). Thank you for your comments on improving this form.

DETAILS
The Texas Department of Transportation (TxDOT) has revised the Form VTR-214 (see attached) to include the following changes:

Disability Area

  • Moved the Disability Statement from the bottom of the form to the top of the form.
  • Moved and bulleted the list of physician’s authorized to complete and sign the form.
  • Added check boxes for Mobility or Non-Mobility Impaired criteria.
  • Moved prescription information to the front of the form.

Applicant Area

  • Clarified the use description section for placards and license plates.
  • Reduced the number of check boxes in the application section.

COUNTY ACTION
If you desire, you may use the existing supply of forms until inventory is depleted. The revised form is in stock at Huntsville and distribution will be made at the time of reorder. This form is also available on the TxDOT Web site at www.dot.state.tx.us, and through
Fax-on-Demand at 1-888-232-7033.

If you require a copy of the original signed RTB and the attachments, please email your request with the RTB number to VTR Info Request/Open Records. Subscribers will automatically receive all RTB mailings as they are issued.
